Applications for Berkeley Math Circle 1998-99

Every student who wants to enroll in the Berkeley Math Circle must review carefully the five forms below with his/her parents, complete them and bring them to a meeting of the Circle.

Since there will be students traveling on a weekly basis to the Math Circle all the way from San Jose and from Davis, parents should make every effort to ensure safe transportation of their children to and from the site of the sessions. In the very unlikely case of a serious medical problem during the sessions, every attempt will be made to contact the parents first, but we must have the forms in case a decision has to be made before the parents can be reached. If any of these forms causes you any concern, please be assured that all of the individuals who will be working for the Math Circle are responsible and caring adults.
