Here are two sample midterms from when Kenneth Ribet taught the course in 2002 and 2005: Sample 1, Sample 2. I think both exams cover chapters 1-2, though Ribet also covered section 2.6. You should also take a look at my practice questions.

Once again, the two sample midterms from when Kenneth Ribet taught the course in 2002 and 2005 are nice examples: Sample 1, Sample 2. You will find the first especially useful, as it seems also to focus on chapters 4 and 5. You can ignore the last problem as it deals with dual spaces: a topic we skipped. Lastly, I wrote up a nice little review of the important topics in chapters 4 and 5. It is in your interest to give it a close read, especially my additional comments about the Cayley-Hamilton Theorem at the end.
